The Case Coordination Unit provides an assessment to all older adults and disabled individuals over the age 18 who are needing to information about home and community based services or Long Term Care Facility Placement. The Care Managers provide the individual with resources in the community that may best fit their needs. The Care Manager will discuss home and community based services such as a homecare said, adult day services, home delivered meals, etc. Other options to be considered are Assistive Living, Supportive Living, Short Term Rehab and Nursing Home placement. The individual and their caregiver are allowed to make an informed choice regarding their care. Individuals going into a Long Term Care Facility must have an assessment completed by a Care Manager.
