Understanding your care program choices can be difficult and confusing. Community Care Systems Case Coordination Units provides assessments for older adults and disabled individuals over the age 18 who need information about home and community based services or Long Term Care Facility Placement. Our Care Managers provide individuals with resources in the community that may best fit their needs.
A Community Care Systems Care Manager will discuss home and community based services such as a homecare aid, adult day services, home delivered meals, etc. Other options to be considered are Assistive Living, Supportive Living, Short Term Rehab and Nursing Home placement. The individual and their caregiver are allowed to make an informed choice regarding their care. Individuals going into a Long Term Care Facility must have an assessment completed by a Care Manager.
